I really like the rename part of refactoring so I wanted to add it to my toolbar. Unfortunately there are no icons so the toolbar ends up growing very large. Any way you could draw some icons at least for rename and create implementation to prevent this from happening? See attached example:

We are considering adding icons for the refactoring commands at some point:

For now, are you aware that if you right click on the toolbar buttons while customizing the toolbar you can select one of the default icons, or create your own simple icon? Not quite the same, but this might help.

Alternatively you can right click on a symbol in the editor and select a refactoring command from the "Refactor (VA X)" sub menu, or hover the mouse over the symbol and click on the down arrow icon to open the refactoring context menu.
